Protected Health Information (PHI)

Any health information collected is treated as Protected Health Information under HIPAA. PHI is accessed only by authorized clinical staff and is never sold or shared for marketing purposes. You will receive a full Notice of Privacy Practices at your first appointment.

Data Retention

Medical records are retained for a minimum of 10 years per Oregon state law. Website inquiry data is retained for 24 months.
