By using the Meridian Health website, submitting a patient intake form, or booking an appointment, you agree to these Terms of Service. Please read them carefully.
Meridian Health is a licensed medical practice. However, the content on this website is for informational purposes only and does not constitute medical advice, diagnosis, or treatment. Always consult a qualified healthcare provider regarding any medical condition.
Appointments are scheduled via our online booking system or by phone. Appointment confirmations are subject to provider availability. We require at least 24 hours' notice for cancellations or rescheduling to avoid a late cancellation fee.
Completing our intake form does not establish a patient-provider relationship. A formal relationship is established only upon your first confirmed appointment and acceptance into our practice.
We accept select insurance plans and offer self-pay options. Copays and balances are due at time of service. Unpaid balances may be sent to collections after 90 days.
Meridian Health complies with the Health Insurance Portability and Accountability Act (HIPAA). Your health information is protected and used only as permitted by law. You will receive a separate Notice of Privacy Practices at your first visit.
Meridian Health is not liable for any damages arising from your use of this website or reliance on information provided herein.
